Orthopedic Surgeon Here. Let's review
(I did not see the original xrays so I am only commenting on these)
1.) Buttterfly fragment- so what. Some studies have shown a decrease in healing with those fractures initially, but he has a nail in. Performing extra surgery to remove the butterfly fragment is absolutely stupid. It is waaaaay too early in the game to know if he is going on to a non-union (6-9 months is the accepted answer for that), so that sort of conjecture is pointless.
2.) Fibula fracture- a double so what. Big debate whether to fix them at all initially. If he had it before and they didn't fix it, definitely no reason to do it now. If he got it in training, again, so what. The fibula bears very little significant weight of the lower leg. If it's painful to him now, yea then sure, slow down the training. But to go back in and cut him open to fix it, on a non weight bearing bone? No way.
Overall I see very little concerning things here other than he may have pushed it too hard early. But nothing a little rest might not fix. Waaaay to early to even be discussing potential second surgeries.
Other than that, the hardware looks good, and I see nothing too concerning on these few radiographs
