You remember that whole "unified rules" push to change scoring criteria and the grounded fighter definition that happened round about 2017-ish, and almost every other event back then, it seemed like the commentators had to talk about whether the state they were fighting in used the old rules or new "unified rules"? (I always think of the Tim Means - Alex Oliveira fight)
I think a combination of a) a more recent announcement of changing the unified rules wording and b) COVID severely hampering the usual diversity of locations for UFC cards got people either thinking it was solved or forgetting it altogether, but that problem is still very much going on. It's not that Canada doesn't, since British Columbia did adopt the new rules (Vancouver cards are fine) but Ontario didn't. So fights in Toronto go by the grounded rule of 'If anything other than soles of the feet are touching.' It even specifies one finger and has an additional statement that even fighters only on their feet, but leaning against the cage enough to support their weight can be considered grounded too
