I've never worked in LE or Corrections. But if you told me I had to protect a very high profile prisoner who had already attempted to take his own life, I would have done something like this,
- I would have dressed Epstein in clothes that couldn't be used to form anything he could use to hang himself with. Same for his bedding.
- The cell would have no anchor points; nothing to fix a noose to, even if Epstein had somehow managed to make one.
- Nothing in the cell would have sharp edges. It would have the bare minimum of equipment, and all of that would be fixed to the floor/wall.
- I'd have at least two cameras running 24/7, every second of footage recorded, with multiple redundancies in case of power failure etc.
Now if a civilian like me can figure it out, why can't the warden and PO's?
- At least two guards on duty at all times. One of them within arm's reach of the cell at all times.
