Assisted suicide is my top choice.
I generally find it unlikely that someone can be observed 24/7 for days or even weeks without any moments of reprieve during which they could kill themselves. I doubt people were tasked to do this on such an indefinite basis, and if they were, I doubt people are actually capable of doing it either. It's an insane amount of concentration, perfect rotations of rosters, etc. for days/weeks and people just don't function this way.
Could it have been murder? Yep, a lot of people would want him dead. But I tend to err to the side of 'incompetence' before 'malevolence' until evidence shows otherwise.
