I have rare shoulders.

Cissus takes a week or less to work. Sometimes I take it before I go to sleep and wake up feeling better. So it depends. I take it consistantly once a day at 1/3-1/2 tspn and if there's a flare up or injury then I bump it up to twice a day and slightly bump dosage...

How common are cases of what you described versus the classic AC shoulder separation?

I don't know. I think osteolysis of the clavicle used to be somewhat rare but I think it's becoming more prevalent since more people are doing a lot of weight training now.

Is heavy crackling, crumpling inside the shoulder symptomatic of scar tissue?

There can be multiple reasons but scar tissue is definitely one. My shoulders do the same shit due to all the injuries I've had. It's just more of an annoyance than anything (for me).

And is that a problem in of itself?

Probably not, but it depends on the cause. It very well could be related to your AC injury.
