During my first playthrough i mentally took note of all those decisions that seemed to hide implications and possibilities of more or less radically modifying the course of events but when i found myself doing different things i realized that the changes were minimal, often only perceivable in a different line of dialogue or in the same events that happened in the previous run but simply anticipated or postponed.
An immense disappointment was the Dark Urge's run: if you have played the first two titles it will appear banal, uninspired and thrown there for pure fanservice (with a sad retcon for those who have played Throne of Bhaal).
It's a great game but as you progress you realize that it's "less beautiful" than it seems at first.
