I don’t believe the island had an airstrip for landing a plane. Everything I read says it needed to be accessed by helicopter or boat.
Here is a snopes article about the accusation.
Little St. James found that guests accessed the island either by boat or helicopter because it didn't have an airstrip, further discrediting the claim that any of Trump or his businesses' planes would have flown to or landed there.

'Retired pilot' claimed Trump's plane flew to Epstein's island 37 times. Here are the facts
U.S. President Donald Trump said in January he had never visited the late financier and convicted sex trafficker Jeffrey Epstein's island.www.snopes.com
From a Microsoft search.
"Little Saint James—commonly referred to as Epstein Island—did not have a traditional landing strip or airport. Here's what we know:

- The island lacked an officially designated ICAO or IATA airport code.
- It did not have a landing strip suitable for airplanes.
- Instead, transportation to the island was typically via private helicopter or boat.

- Epstein used a private helipad on the island for air travel.
- Guests often arrived by private jet to nearby airports (like St. Thomas), then transferred by helicopter or boat to Little Saint James."
