If a non-binary person is pregnant or may be, but only "M" and "F" exist on the form, the Doctor may not have all the information to render treatment up front.

This is a case of YOU overlaying your personal politics on something that is outside the political spectrum. Feelings before facts. And yes, not medically accommodating trans people in politically neutral things like medical forms is a way to have exclusionary policies in society.
