I think you're mistaken about that. It's considered to be elective surgery and insurance rarely covers it. They typically have to pay out of pocket for it. Many get the money from sex work actually.

I dont think it can be done via private system, simply because big entities, which you must be to be an insurer, are bed buddies with our government. Any gigantic entity is susceptible to this due to the donation system that exists with all sorts of favors.
