- One of the most realistic looking films, so much that upon viewing, Charlie Sheen called the police to report it as a snuff film. A woman is found tied to a bed and tortured in the most vicious manner possible.

06. Cannibal Ferox (1981)
- A vicious film, similar to Cannibal Holocaust, where a tribe takes revenge on friends who tortured and enslaved cannibals.

05. Cannibal Holocaust (1980)
- The most banned film of all time. The director had to bring his cast to court to prove they were alive because of how realistic it was. He went to jail as well because the animals killed in the film were real. The king of found footage films. The director also had to convince the courts that one of the natives in the film wasn't actually murdered by proving how he made it look so realistic.

04. Salo or the 120 Days Of Sodom (1975)
- Often regarded as the most disturbing film of all time, although lacking gore, this film is easily one of the hardest to watch. The director was murdered right after the film's release. As four fascist libertined round up adolescent boys and girls and subject them to 120 days of physical, mental and sexual torture. The dialogue plays a huge role in the movie. The way its so casual, makes it all that more disturbing. Rape, murder, everything you can think of.

03. Santa Sangre (1989)
- An artsy film with a well deserved NC-17 rating, where a young man in a mental hospital has flashbacks of his life as a circus performer with his family, where he saw his father commit acts of violence and this leads to scenes of nothing but grisly murders.

02. Island Of Death (1976)
- Another film that lacks realistic gore, but is easily the most bizarre film ever. Including everything from sex with animals, incest and various other hanious acts. A british couple on vacation, travel to a Greek Island where they will unleash hell on the islanders by basically putting them in situations where they commit sin and therefore justifying their murder. For example, the woman could have sex with a married man, and then she and her husband would have reason to kill this man for cheating on his wife. A strange flick, with a bizarre twist ending that makes it even more disturbing. Deranged thoughts also are a huge part of the film... for example, having sex with an animal, then killing it so that it doesn't tell anybody about the sexual encounter.

01. Schramm (1993)
- The most disturbing film of all time because of its realism. A look into the mind of a serial killer, from the killers perspective. What makes this movie so disturbing is that despite it being disturbing, you also may find it sad and tragic and actually feel bad for the killer, who is based on various serial killers. The tagline for the film quotes, "Today I am dirty, but tomorrow I will just be dirt.", a famous quote from real killer Carl Panzram. The movie is about Lothar Schramm, who has fallen and is dying in a pool of his own blood and horrific flashbacks of his miserable life flash before him in his final moments. He is an lonely cab driver who does nothing but run track, make an honest living driving his cab and invites callers into his home and then slits their throats, arranges them in suggestive poses. His sex life is non existent, besides intercourse with his inflatable dolls, his social life is non existent. He is infatuated with his neighbour, but his beautiful neighbour is a prostitute which whom he is smitten, and is forced to masturbate over her unconscious body spitefully when he gets the chance because she does nothing but ignore his desire for her. Leading him to self harm, including nailing his foreskin to tables. He is plagued by paranoid delusions of his leg being amputated, getting his eye removed by his dentist, vaginas with teeth and various other horrific delusions, brought on by his mental illness which is made more severe with his horrific life that seems to not get any better over time. A tragic and realistic look into the mind of a deranged killer who is a victim of mental illness.

Have only seen three of the movies listed, but if they're are 8 more fucked up films than August Underground I'll pass.

I've never understood the appeal of these types of movies. Also, I think for anyone that has journeyed into the deep web, these films wouldn't shock anymore.

Who the fuck could enjoy a movie with the sole intent on disturbing?

almost all those films are not shock films... the ones that are just pure shock films are a serbian film mainly, but just the content of that one makes it deserve to be in the top 10.. human centipede is a perfect example of a shock film.

As others have said Requim should be on there. Disturbing but still a good movie with people who can actually act.
