In Epstein's case, he would have associates, which we know at the very least included Ghislaine Maxwell who was still free at that time, plus mossad if you believe he was a mossad asset, likely some other staff, who would all or some have custody of the incriminating evidence against the people being blackmailed that would be released if they rolled over on him or didn't do what he wanted.
And what's more, you can even do this with people who aren't even "p words". You just do exactly what he did do, which is have a revolving door of 18-25 year old employees as masseuses and house hands who you pressure into banging you, sprinkle in a handful of 16 and 17 year olds, give them drugs and alcohol, impress them with money and introducing them to rich and "powerful" people, then put cameras all over hell to film it all, then tell the person you're blackmailing after the fact that the girl was underage and you have video of it, which you can do even if she wasn't one of the underage ones because how the hell would they know.
What you don't do is just blurt out after meeting another rich guy "hey, here's my number if you ever wanna come over to my house and fuck some minors", then when one of them outbids you for a property you wanted, then throws you out of his club, disowns you as a friend, has the power to pardon you before, during or after your arrest and doesn't, so you just decide "that's ok, I'm already screwed but his secret's safe with me".
