What looks like an extra toe is just the shape of his foot - and it looks extra weird due to the camera angle and shadows (lighting and angles, bruh)

Could be a tailor's bunion/bunionette seen head on with tricky perspective. The shadow from the pinky makes it seem like a separate toe, but in reality it looks like that section is behind the pinky and might be the metatarsal sticking out:

Could also be that his feet are normal, and it's just the foreshortened angle from the front side, the 5th metatarsal naturally protrudes a little, especially when the fatty tissue and skin press against the floor and bulge out:

He might also have polydactyly, but that's probably the least likely scenario, it doesn't look like there's a toenail there. The incidence of polydactyly of the foot is 1-2/1,000 births, so not that uncommon. Should be easy to see in other photos. If that's the case, book him vs the 16 Toe Kid in a battle for toe supremacy:
