The whole thing was a sham from the get go. The TUE exemption was a clause that fighters near and far saw as an opportunity to get an advantage, plain and simple. Due to the UFC and MMA's increasing testing abilities for PEDS anyone on gear needed to have this. Heres why: CIR testing capabilities. Carbon Isotope Ratio, a test which could detect the presence of synthetic testosterone.
So fighters would cycle off before a test, show low T, get scripted, and apply for the TUE. Once granted it was basically a get out of jail free card to ramp up testosterone use during training camps. So long as fight night the were at or under a 4/1 T/E ratio. Most humans are 1/1- 2/1. Of course they would pop hot for synthetic testosterone..but of course, they had a TUE. No harm, no foul.
Fighters without a TUE couldn't piss hot for ANY synthetic testosterone and also had to be under the same 4/1 T/E ratio as the fellas on T. Where some of them using Test as well..you bet. Most used Testosterone Suspension. Pure testosterone not suspended in any sort of time released ester. You shot it and in 15-30 minutes you were ready for war. Positive's was quick acting and out of the system in 4-12 hours, the leaner you were the faster it would leave the system. As it wasn't suspended in an ester it was also mg for mg 100% test. Where test cyp is 70mg test 30mg ester per 100mg/ 72/28 for enth. The downside was depending if it was suspended in water or oil it could be painful to inject.
Of course many fighters where using other agents as well and then letting them clear out before the fight. The TUE was just a major loop hole the UFC eventually put the kibosh on. For the early days of NHB/MMA it was a circus, once it became mainstreamed things changed quite a bit. TUE was really only being used in MMA and it brought a lot of negative light to the UFC and sanctioning bodies. Left in place everyone and their mother would have a TUE!
