I'll take a "stab" at it (pardon the pun). It's a combination of two things:

1. this guy's leg was mangled in an accident at some point and he had sloppy surgery. If you look, he doesn't have hair on his leg in the area where the deformed area is. That's not simple acute inflammation.

2. He filmed involuntary twitching of his inner Gastrocnemius muscle. Its called myoclonus. He probably had nerve or even spinal injury (see 1 above) or drinks too much and damaged his liver.
