Secondly, the story is that he has been staying awake for days at a time and believing there are chips in his leg, people inside the walls, wife is vampire, etc.
It seems pretty obvious that he's been smoking meth/crack, slamming powder, or something of the like.
At least that is more probable than sudden onset of crippling mental illness at age 35, which is almost unheard of.
