isn't it a lucha libre mask? if so highspots.com has a good selection of them, not sure exactly which one Mayhem wears, but you could get one somewhat like it.

This is the mask he wore when he was going to fight GSP. It is used by one of the members of Slipknot.

 

Well that's it. Man I had no intrest of getting one but only $29 and that thing actually looks pretty sweet. I just might sport something like that while warming up before a fight.
