Lacunar infarctions are a specific type of small vessel disease where you get occlusions of deep branches of major cerebral arteries. Then he says that vascular dementia is a form of dementia with Parkinsonian features that wax and wane depending on blood flow. This is an inaccurate description. He says she couldn't have gotten by with Parkinson's but could with Lewy body dementia because it waxes and wanes. The waxing and waning is more changes in cognition and attention. People with Lewy body often first get misdiagnoses as Parkinson's because they share similar features and can often look the same. But Lewy body disease differs clinically by the predominance of dementia occurring early in the disease where it typically develops later in Parkinson's. People with Lewy body dementia also often have visual hallucinations, auditory hallucinations, paranoia, and delusions. I would think it would be harder for Hillary to hide early Lewy body disease than early Parkinson's.

This is the part that makes me think that the guy writing this might possibly not be an internal medicine doctor. If he is one, he is either trolling or dumb. I'm sure Clinton's doctor had a high suspicion of pneumonia from the history and physical exam, maybe started her on an antibiotic at the time to get the ball rolling, and ordered chest imaging for confirmation. Routine lab testing to establish an etiology is unnecessary for outpatients with community acquired pneumonia. Antibiotic treatment is empiric. It's uncommon to transmit pneumonia between people, and after starting an antibiotic, the risk drops off to nothing.
