That's the reality of physical inactivity ; and keep in mind he's still a young man.
It gets even worse as you get older. I often saw old people in nursing homes, still able to walk perfectly (although slowly, obviously), fall someday and break their leg, typically femoral neck.
They get hospitalized, often with surgeries, arthroplasties ; and because of that, they are stuck in bed for so long that their muscle mass melts like ice under the sun.
Recovering the ability to walk after that is either a grueling process, or plain unrealistic.
From there, a lot of those old folks just let themselves go, to try and leave this world, because of crushing depression, other health problems, and the terrible feeling to be a burden for their family.
Sorry for that off-topic, but if I can recommend one thing : don't let your old people walk in shitty worn out shoes/slippers. Nothing worse than losing your autonomy.
