The absolute best interpretation of this is that the white South African with Nazi grandparents (sympathizers, whatever) is an unaware dork who somehow didn't consider that he was giving one of the most well known salutes of all time. It's possible. It's also possible that that person who's connected to a president - who wouldn't even condone (Edit: condemn lol) white supremacists - who has a, shall we say, checkered history, just intentionally did the Sieg Heil and tried to cover his tracks by saying his heart goes out to people to give himself the thin veneer of plausible deniability after pissing off Trump's base over the visa situation. Like it or not, Trump has a sizable chunk of supporters who, shall we say, wouldn't be terribly offended by a Nazi salute and are so balls deep in the cult of personality that even if they don't sympathize they'll still defend. It's probably all coincidental though.
