Despite Hendo beating Bisping in a supposed title eliminator, Hendo proceeded to not fight for the title, and instead left the UFC to go to Strikeforce.
By further example, the UFC formally announced that the winner of Shogun vs Vera in 2012 would get a title shot. In fact, they never gave Shogun his promised title shot after beating Vera.

You are right that sometimes the UFC didn't follow thru. But in fact the plan was changed before the event so that isn't a case of not following thru.

- Main event vs. co-main event: The initial plan was for the winner of the main event between Shogun and Vera to get the title shot.
- Changed decision: Dana White changed the plan and said the winner of either the main event or the co-main event could get the title shot.
- Both fighters won: Both Rua and Lyoto Machida, who fought in the co-main event, won their respective matches by knockout.
- Lyoto Machida got the shot: Despite Shogun's win, the UFC gave the next title shot to Machida, which left Shogun to re-enter the division's general pool of contenders.

Funny enough none of them got a shot after that.
An actual title eliminator requires the UFC to build their fighters thru proper match making. An excellent example was Sonnen vs Bisping. Sonnen took Anderson to the 5th and was less than 2 min from taking his title by lopsided decision. He then fought Stann (who had he won is positioned as a possible contender) and then matched up with Bisping who was on a 4 fight win streak and was 5-1 since he lost a title eliminator to Hendo. His overall record was 22-3 and he was 8-2 at MW in the UFC losing only to Hendo and Wand.
It was legit to have the winner regardless of which one challenge Silva.
The 4 man tourney as it were of Shogun, Vera, Bader & Lyoto made no sense. All 4 were finished by Jon between March 2010 - Dec 2011 with Lyoto being the most recent. He in fact made the least sense of all.
Since losing to Jones:
Vera:
had a NC (loss that was changed) and a win over Elliot Marshall before losing to Shorun.
Shogun:
had a win over Forest and a loss in a FOTY to Hendo before beating Vera
Bader:
lost to Tito, beat Jason Brilz, beat Rampage before losing to Machida
Machida:
Had no fights aside from beating Bader.
Bader would have made the most sense with 2 wins and Rampage being one of them. He should have been matched with Shogun and Machida with Vera IMO. Had Bader won you could at least say he had time to evolve since last fighting Jon. Lyoto had lost to Jon only 8 months before the UFC on Fox 4 event and had no other fights let alone wins.
