Can someone explain why some MMA fighters struggle to pay for their medical bills?

I mean, let's say the UFC doesn't take care of them because they aren't that big of a name or have been out of the game for a while, surely as a professional MMA fighter you have health insurance? If you don't, you're basically an owner operator trucker without automobile insurance. Surely, any fairly decent or even average insurance covers medical procedures for fighters? Why do we constantly hear about fighters raising money for procedures?

It's basically glorified worker's comp insurance. It only covers fighting related injuries.
