Anthony Johnson not an option? Dude was fighting at welterweight and came in 11 pounds overweight to a middleweight fight.
 
Joe Riggs also missed weight for a UFC title fight
 
Lutter because he went through TUF for nothing.

Didn't Kevin Randleman slip and fall in the sauna knocking himself unconscious before a title fight/defense? I think that deserves an honorable mention.

I have no idea why Riggs doesn't get as much heat as Lutter does for this, I actually thought of Diesel when I was the thread title. I think both have finally been eclipsed though because at least they earned their title shots and fought after their blunders. I mean finishing Lytle and winning that season of TUF were far more deserving than an immediate rematch after losing your title in a prolonged, lopsided beating where you get finished.

Hyun Gyu Lim was supposed to make his debut on the Franklin-Le card but had to pull out due to his weight cut. There was a report that said him and his team wanted to try again after he was taken to the hospital, but the UFC doctor said he could die if he didn't hydrate himself. Not sure how legit that report was but according to Lim:

H.G.L.: My body just couldn't handle the cut. The day before the weigh ins, I collapsed in the sauna. People are saying that I almost died, but that's a pretty big stretch. Even immediately after I collapsed, they took me to a hospital that was right next door to the spa in the hotel, and all my vital signs were normal. I normally cut quite a bit of weight, but it was the first time that's ever happened to me.

How about the Ken Shamrock/Kimbo/Petruzelli debacle, it would be great if Soto managed to replicate that type of feat for a title.

Riggs was a replacement for someone else after having fought only a month earlier. I don't know when exactly he was penciled in as the replacement, but it seems like it was a couple of weeks out. That's probably why
