That’s actually not true. Steroids account for only 10-25% of the cases. Both of these studies say this.

I'm not sure, but I think that USADA tests urine (and blood?), but does not do visual inspections. @dim_ .

USADA testers themselves do not do physical examinations of the athletes for signs of doping. Not in the sense that a needle mark would lead to a violation anyway.

But, USADA do "intelligence based target testing" and intelligence could be (im talking generally in all sports) anything from an anonymous tip, sudden performance improvements, to visually obvious physical changes.

But that would be solely triggers for target testing. Any sanctions against an athlete will generally be 100% science (ie. analytical adverse findings) based. (There are occasional exceptions when its the results of investigations where there is a paper trail or other evidence)
