I believe the theory is that the struggle to control and balance the unstable situation exercises more and different muscles.

Which really only makes you better at performing that particular unstable movement. The literature I've read shows little to no carryover into other movements, activities of daily living, or athletic performance which is why I don't use unstable surfaces with my patients.
