Youre asking way too much of a ref to determine where a fighter was aiming his strike exactly. Ultimately it's simple you throw the strike, you're responsible for it.

No because turning your back is foul. Its effectively impossible to hit someone in the back of the head at striking range and it be a foul on the person throwing the strike.
