I wonder if a needle in the lung is the big concern. They do needle biopsies all the time with a much larger needle than would be used to inject a painkiller. What was being injected into the lung is probably more of the problem than the puncture. They usually inject in multiple places so it the lung was punctured several times it might be serious.
