In the UK, a Surgeon is referred to as Mr/Miss/Mrs. This is because back when Surgery was becoming a specialist field, other Doctors looked down on them. Because they, "only" cut patients open. Thus, Surgeons were not called Doctors by GP's. Even today, the tradition continues.

I always referred to my Surgeon as Doctor, on the basis that it's a good idea to be polite to a man who's going to drug me and cut me open.

Given the amount of training they have to do, and the pressures of the job, they earn every penny, IMHO.
