I think he wants something to happen to Buffer's vocal chords so he can't be the announcer, but doesn't necessarily want anything bad to happen to Buffer that would jeopardize his overall health.

A common ailment that's not life threatening would be polyps on the vocal folds. Common in singers and people that talk a lot. Usually, if they're bad enough, the doctor will recommend vocal rest to reduce inflammation and then they'll perform surgery to remove them. Most folks make a full recovery, but repeat surgeries and build up of scar tissue can definitely affect your voice in a negative way.
Unfortunately, Buff is a professional. I'm sure he's had vocal coaches and knows how to project his voice without straining. He probably does all the other things that professionals do, like properly warming up, staying hydrated, and resting his voice before and after events. His brother is still going strong in his 70's despite having throat cancer, so I don't think Bruce is going anywhere anytime soon. He does sound strained from time to time, but overall, he still has his chops.
