I'll answer the last one - no. Or at least I can vouch that you can go - at least - four years with balls evolving through various shades of blue (all the way to necrotic black) and come out (hrhrhrhr) fine on the other side (college wasn't exactly packed with action for me).

You don't need a belt just the section that has the loops work. I was just using belt line as a general term. Also wear a condom.
