How exactly do I "make" a Custom Belt?

I understand how it works. I have the years in and the number of posts, but I have no idea how to actually make a custom belt. I have a couple of ideas in mind for myself, but if I can't make them, it's pointless.

Where do I go to make them?

I would use Photoshop if it's a JPG and a GIF editing software for a GIF.

PM an Admin sending them the belt. Mods don't have the right to put custom belts, only Admins.

The dimensions for a custom belt are 100 x 50.

The max filesize is 10kb.
