For me, it was Ecco the Dolphin, for various reasons:

The music was very eerie and the sound effects were painful.

The deep blue sea was just scary, waiting for something to jump out.

His family were taken away from him as soon as the game started and this poor killer whale lost his pod.

Certain sea creatures are monstrously scary anyway

Other scary creatures included alien/larvae things - I used to think this was a 'willy monster', it would chase Ecco.

I watched my dad play the original Doom and that never bugged me but for some damn reason the original Prince of Persia made me uneasy. It had an eerie vibe.

Goonies II freaked me out. I dunno if it was the music or what. Friday the 13th on NES too.
 
The timing of events certainly had a lot to do with it, but I was about an hour and a half into Resident Evil Director's Cut when it first came out, and my friend butt dials me. This was the friend who was an early adopter when cell phones first started to take off. I don't have a cell phone, I don't know what's going on, it's late at night, and it sounds like Satan himself has just called me while I'm playing a horror game. It certainly made the game more interesting.
 
I had this old 'adventure' game on the Sega Saturn called "D" where you play this woman who's father has gone on a mass murder spree at a hospital. The police send you in to try and talk him down, and things get weird from there. Once you've entered, the hospital has turned into a hellscape, and you have to solve puzzle so you don't end up flayed or impaled like all the patients. To top it all off the damn game had a timer you had to beat it in. Oh, and it turns out your dad is Dracula and you ate your mom or something

Silent Hill 1 creeped the hell out of me when I was little.
 
The Suffering, one of the best horror games ever made. It's an older game but still impressive. The sequel, "Ties that Bind", sucks though.

 
First creepy game I played as a youth was a point and click game on the pc called "phantasmagoria". It was shot with live actors and the cut scenes were just brutal murders and even a rape scene. The end was unnerving as you had to run from the killer/monster and if you got caught your head would get split open. Looking back now the deaths had super cheesy graphics but for being like 12 at the time the game had me creeped out.

Also silent hill 1. I legit couldn't play with lights off. God was I relieved whenever the world turned back to normal and the music mellowed out. The game always changed camera angles so when you walked into rooms you couldn't see what was coming but you could hear it.
 
Those quest games always scared me. Especially police quest 2 when you are tracking down Harold baines at the park and get shot .
 
Duck Hunt. Not being able to shoot that snickering dog drove me crazy.

Some sick fuck actually made this game and for obvious reasons it is very disturbing ....for those unfamiliar you recreate the Kennedy assassination by being a shooter and killing the president.....
 
Not much really disturbed me as I was a kid in the 80's and games weren't that good yet. I remember thinking the game Narc was kind of gnarly, because there was body parts flying everywhere and guys throwing drug needles at you, but that's about it.



There was an arcade game out back then called chiller where you would shoot helpless people that were locked up in torture devices. This would have disturbed me, but I never heard of or saw this game until I was an adult.
