5. A screenshot if possible
6. Brief description of how the ad is a problem
7. Page/Pages affected
8. Date and time of occurrence

    TTT for this. I've seen tubgirl about a dozen times in the last 12 hours and it would be much easier to get rid of if we could see which mods were online to PM.
