Anyone been thrown out of the cage?

Was reading the UFC official rules and saw the XXVII rule of fouls denoting it is forbidden for a fighter to throw the other fighter out of the cage. Has this happened in the UFC or some other promotion? I'd figure the rule stems from an actual real life situation.
