This still isn't very clear.

Did they cut him for not notifying them of the surgery soon enough?

I recall an interview with Silva and Shelby where one of them said something about how even if you don't intend on pulling out of the fight and think you'll be healed in time they still want to know so they can have somebody in stand-by if the worst case scenario comes to pass.

Otherwise it just fucks them around and you end up with cards like UFC 177 where they didn't even have enough fights for the televised prelims because dudes like Odoms pulled out.

I was honestly excited for Odoms debut, he's been around a while and he deserved a shot. It sucks that he probably won't get another shot if Konstantin beats him.
