Look at the list of 100m world record holders. You have to get down to around the 50th spot before you get to a guy who hadn't been busted, and isn't named Bolt. One of the medal winners in weightlifting in the London Olympics actually finished 9th. And that guy failed at test at the Rio games. Look at the Tour de France wiki page on doping. There are multiple years where none of the to ten had a clean history, and few if any recent years where the top ten were all clean (so far).

You have to work damn hard to get close to the top of any sport. But it's almost required that you take something for that last 1%.
