Outside of UFC you also have Gabi Garcia very recently overstepping the weight limit mark by an obscene amount. The granny scheduled to fight her was having none of it.
It was actually quite funny, especially if you know how Rizin is like.

the most significant time to start from would probably be UFC 214 when new rules took place for weigh ins .. Mid 2017 .. And then maybe backwards same amount of time

this is a good start point though .. We've had quite a few instances already this year

Iaquinta did NOT miss weight.

He made weight for the fight he was contracted to fight then stepped in to the title fight and was never given an opportunity to make 155.
