Why doesn't UFC/ USADA go back and test for EPO on all fights since ufc 189

That would be massively expensive and take forever. What's to say they even still have all of the samples? USADA was notorious in boxing for losing samples or flat-out not testing them.

At the moment it costs a bit more then 4x's the cost of the default test. And thats just for EPO nothing else.

And the other bit you just totally made up. Bearing in mind usada dont actually have the samples it would be pretty hard for then to lose them
