At first ..... cm punk ???

after that a combination of Roufus( come on - he HAD TO know he had a candy cane) & UFC- the first fight after all that prep - nada ..This cat had no business in any cage, let alone the UFC. It was obvious he wasnt a fighter / mmartist
 
The fault is in CM Punk and his training. They gave CM Punk every opportunity to secure a win, it was up to him to do it and he didn't.
 
The UFC made money off of CM Punk's fights and provided lolz for hardcores in between cringes, so I wouldn't really call it a disaster from the UFC's perspective. But promoter makes the fights, so if you must blame someone blame Dana. CM was just a guy living his dream, albeit a delusional one.

You missed of couple of other suspects:
Brock for creating a myth that transition from WWE is easy for fighters and lucrative for promoters
Ronda for proving that a draw is a win no matter what
 
All of the above.

Although I blame Dana least. His job is to generate income for the UFC, and all indications are that CM Punk brought in PPV buys. Can't really blame Dana all that much for signing a guy with a big following.
 
If cm punk brings that knee up vs. Gall, we are having a completely different conversation. He would have been the original flying knee KO poster boy. Jorge would have been blamed for for copying Phil’s knee. But he didn’t throw it, not in this timeline of the multiverse. But you can bet there is one version where he did.
