Because the old rules still apply
A fighter is grounded when anything other than the soles of their feet touched the canvas; a knee, a hand, a finger. A fighter can't be legally kicked in the head when grounded.
Yet "accidental knee on a grounded opponent" was called, when those were deliberate.
