I think he simply wanted to be honest with the public regarding his health. I see no reason to believe hes ducking or wanted a way out. This is how they put food on the table.

Don't you think that Cain is wise enough to know by now what actions would likely take food off of the table? C'mon, tend to hide all sorts of injuries and consumption of banned substances from the auspices of the commissions, but Cain went full bore in his revelations. There was only one reasonable action the commission could take, and I have no doubt that Cain knew this. He's been involved in the game for too long not to know.
