Was just rewatching Weidman/Munoz and my god was that bad

So what are some other instances where the ref messed up badly?

I can't remember the fight, but I remember Kim Winslow (or at least my memory tells me it was her) letting a fight go so long, the fighter actually looked up at her to see if she was going to stop at...as the other fighter isn't defending himself and just getting pummeled.

When Faber choked out Mizugaki the ref was so out of position I thought Mizugaki was dead when Faber let it go.
