Here they can't ask until after five days have been taken. Then they can ask for a Doctors note if they feel inclined to do so.

Yes, I'd really say something that dickish, I'm over biting my tongue with asshole supervisors.

With that said, they are not allowed to ask questions like that. At most they can request a doctor's note.

Now to be fair, I don't know if this is one of those annoying employees who extends their holiday or vacation time by calling in sick, so I won't completely demonize the supervisor in this situation.
 
Just write as little as possible.

I have a respiratory infection.
Or I have a stomach bug.

Depends on the country. In some countries, I don't think they can ask in that way. In which case, I wouldn't even respond. I would probably talk to HR instead.

In other countries, I would respond with a vague description of the illness.

Regardless, with that kind of communication, I would find a new job or at least a new boss.

This. I accumulated slightly over 100 sick hours at my company. Went to HR and they told me that without a note from a doctor I can only take 40 hours (5 working days). So after that I went to a primary and got a note so that I could use remaining 60 hours.

How would you respond?

For the first 30 years at my last job, we got paid for all of our vacation time for the year in April. Then you could take time off or not. When the company was sold, the new owners paid vacation time when you took time off or paid what you hadn't used at the end of the year. Then they sick days and vacation days into paid time off so the two were interchangeable. The first two years we had to use at least 1/2 of it then the rest could be rolled over into the next year or we could get a payout for it. They went from the anniversary of the day each person was hired and changed to January first so everyone's PTO expired at the end of the year. Next they decided that everybody had to take time off instead of rolling it over or getting a payout so at the end of the year everybody wanted days off to use up all of their time. We had to submit a request for time off. I got 160 hours per year and I had another 200 hours rolled over from the previous years. I kept submitting requests for time off to use it up and the manager kept denying the requests. HR contacted me about it in September and I told them that I was trying to take time off but kept getting denied. I ended up taking Fridays and Mondays off for the rest of the year so every weekend was a 4 day weekend plus some other days.
