5. A screen shot if possible,
6. Brief description of how the ad is a problem.
7. Page/Pages affected
8. Date and Time of occurrence.

    To add an avatar you have to press the red x in the top right corner of your screen, then unplug your computer, go to the top of a tall building and throw it off the edge, if there are no tall buildings available, than it's possible hitting the computer with an axe could have a similar effect
