Seems like every fighter has it nowadays and nobody bothers to get it drained

"Drained" lol. That's not how that works. That's permanent damage, and any attempt made at shrinking will leave the skin tissue incredibly fragile and open to falling off.

What method of draining have you seen before? Using a knife?

I thought you could prevent it by draining it right after it happens

When the ear first gets damaged and starts to fill up with blood, that's the time to drain it. If you wait long enough, then yes, it becomes hardened and permanent. But even after draining the ear still looks fucked up and can easily fill up again. It's a pain in the ass.
 
It blows back up the next day at training.
I work with a guy that had surgery to fix it. He walked around with bandages on his ears a few weeks. But yeah- I would think it will just come back if you start training again. (I think he was done)
