#4 is too rampant in most clubs i'm near.

I've been lucky to not experience any bad instructors. My instructor teaches like 4 classes a day, competes at tournaments (black belt), invites other people from academies to come train with us and is all around just a great instructor and person. Only time he hasn't taught classes have been when he was out of state with some teammates to compete.
