i don't think you can be so sure about that. There are lots of fringe and edgy directors looking for realism who might 'allow it' as long as no can prove it. How would anyone know if under the covers they were faking with pasties on or actually penetrated? A good actor/actress should make it impossible for you to know.

But yes on the vast majority of sets, particularly big budget, big name producer, director, etc I would say it would not happen.
