Because the fights are listed on a card or sheet for the judges and athletic commission.
 
King Pun remains the best poster on sherdog and it's not even close.

It refers to the sound fighters make when absorbing a body shot "CCRRRDD"
 
It goes back to the 19th and early 20th centuries, where at formal dances, women would have a "Dance Card". This was a list of with whom they would dance with throughout the evening.

The phrase transferred to boxing parlance, and mma copied it.
 
Why is it called a fight camp? There aren't any tents.
