Worst career collapse after title shot?

After seeing Gabriel Gonzaga at the very bottom of UFC Rio, it got me thinking about who has had the worst collapse after losing their title shot. Going into that fight w/ Couture, everyone at the bar besides a few Couture diehards "knew" that Gonzaga was the next HW champ.. And now he is the opening fight for the prelims..
Anyone else come to mind?

Andrei actually went on a pretty decent run outside the UFC up until he fought Fedor, then his career started going down quickly.
