think I’ve spent a little better than 2 years off the mats with injuries/surgeries…which, now that I think about it, is fairly annoying.
What exactly happened?

What happens to everyone when they do physically strenuous activity for years on end. Various injuries ranging from small setbacks to serious ones requiring surgery.
The real question is, can you get to black belt without them?

my guess is torn ligaments. Those usually require extended lengths of time depending on the injury.
Broken bones are not as bad as ligaments , hate to say it

If I remember, it was elbow surgery in 2008. I'm not sure if he's had anything other than that, but elbows and knees take a while to heal.
